We welcome referrals from Hospitals, Physicians, Assisted Living and Group Homes, Families, Social Workers, and Friends. All referrals must be approved by a physician to ensure the highest level of care. Once approved, a registered nurse and/or therapist will visit your home to conduct a thorough evaluation and assessment. From there, a personalized plan of treatment will be created, discussed, and reviewed every 60 days in collaboration with your physician and the dedicated team at SCOF Healthcare Providers.
